A deadly multi-vehicle accident occurred on Wednesday at 11:34 a.m. on US-412 Hwy, claiming the life of 84-year-old Peggy Patricia Nelson of Westville, OK. Three others were injured in the crash.

According to Siloam Springs Police, Nelson, driving a 2010 Traverse, failed to yield while entering westbound US-412, triggering a collision. A 2023 Freight truck, driven by 26-year-old Jose Angel Prieto of Amarillo, TX, swerved to avoid Nelson’s vehicle but struck it on the driver’s side.

The truck then veered into oncoming traffic, colliding head-on with a 2019 Equinox driven by 79-year-old Janie S. Brown of Siloam Springs. The truck continued off the roadway, coming to rest on the north side of the highway.

Nelson was pronounced dead at the scene. Passenger Evelyn Sue Nelson, 64, of Westville, OK, Prieto, and Brown were injured and transported to Washington County Regional Hospital.

A fourth vehicle, a 2025 Tundra, was also involved in the collisions.

The crash occurred in clear weather amid heavy traffic.

The investigation is ongoing, led by Officer Edgar Galvez-Lua of the Siloam Springs Police Department.
